Service Description: Supports for people living with prediabetes and diabetes type 2 * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * certified diabetes educators, including registered nurses and dietitians * operates across multiple locations in Lambton County, including Forest, Watford, Kettle and Stony Point First Nation, and Sarnia
