Service Description:

Supports for people living with prediabetes or diabetes * one-on-one in person, virtual/phone or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * in person consultation is offered in Chatham, Blenheim, Walpole, and Wallaceburg
