Supports for people living with prediabetes or diabetes * one-one-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, dietitian
Additional services:
- insulin starts
- insulin pump program
- gestational diabetes(IGT)
Mental Health Diabetes Outreach Clinic - Nutritional counselling for people living with diabetes from a registered dietitian and mental health nurse practitioner * offers group educational sessions serving people with mental illness by offering social support and education (Chatham-Kent Mental Health Network Building * last Monday of the month)
