103-bed long-term care home * residential care for 53 private rooms, and 50 basic rooms * 24-hour nursing and personal care * access to a doctor and other health professionals

- medical supplies and equipment (such as catheters, ostomy bags, or wound care supplies)

Meetings:

A multi-disciplinary team conference is held with the resident and/or substitute decision maker(s) approximately 6 weeks after the resident moves in to the home, to review his/her plan of care. A similar meeting will be scheduled annually thereafter. |
