Service Description: Promoting youth, family and community health through service, community education and advocacy * The centre is staffed with physicians, nurses, nurse practitioners, counsellors, a psychologist, dieticians, health promotion and outreach workers

Primary health care including medical and counselling for youth ages 12 - 24

Helping young people break free of harmful habits and behaviours, not just for clients in crisis * Through education and information, valuable life skills are shared that every person approaching adulthood should know.

Fees: Most medical services covered by OHIP * some fees for other programs
Application: Waiting Period: Varies depending upon the program or service * Usually 4 to 6 weeks waiting period
Eligibility / Target Population: Youth between the ages of 12 and 24 * Assisting adolescents, youth, homeless and individuals at risk of being homeless.
Diabetes in Action offers education regarding self managment of Type 1 diabetes (individual and group) and Type 2 diabetes (individual)
