Service Description: Medical diagnosis of work related injury, illness or disease, independent medical support for workers compensation claims, medical surveillance program, prevention of illness and injury through the job and workplace assessments and health and safety education through seminars and workshops

The clinic services are free of charge to workplace parties

Fees: Must have valid Ontario Health Card & Social Insurance Number.
Application: Self, physician, union or company referrals all accepted
Waiting Period: First appointment within 2-3 weeks
Eligibility / Target Population: Any worker who feels he/she may have work related illness or injury or who's workplace is at risk of causing such condition
