Service Description: Information, education, support and programs for those living with Alzheimer's disease or other dementias and their care partners

Education Programs and Resources - Education, awareness programs, and resources for the general public and health care providers on Alzheimer’s disease and dementia

Caregiver Support - Individual support for care partners who are caring for someone with Alzheimer's disease or dementia * delivered by Social Workers / Social Service Workers either in person or over the phone

In-Home Respite - Relief for care partners * delivered by Personal Support Workers in the home of the person with dementia * days and times are flexible * respite relief may include homemaking, personal care, light housekeeping, monitoring, supervision, activation and attendant care

Early Stage Support Group - Support group for those living with the early stages of Alzheimer's disease

Care Partner Support Groups - Peer support groups for care partners * each group has a set focus and incorporates support and education

Mindful Music - Music lending program where clients are engaged through personalized playlists suited to their musical tastes

Inspirational Memories - Experiential art program for persons with dementia and their care partners

Minds in Motion - An exercise and social recreation program for those living with early to mid-stage dementia and their care partner * registration is required

Finding Your Way - Helps people living with dementia, their families, caregivers and communities to recognize the risk of going missing, be prepared for incidents of going missing, and ensure that people with dementia can live safely in the community

Behavioural Supports Ontario - System Navigator - Coordinates services to meet the needs of older adults with responsive behaviours associated with dementia, mental health, addictions and other neurological conditions while living either in the community (including hospitals and retirement homes) or in long-term care homes

Fees: In-Home Respite: $14/hour * Minds in Motion: $20 per 8-week session * Other Services: No fees
