Service Description: Community health centre that supports the vulnerable population in Windsor - Essex * 6 physical sites throughout Windsor and Essex County
  • Ouellette Avenue
  • Street Health on Pelissier Avenue
  • Diabetes Wellness on Luazon Parkway
  • Leamington Site on Princess Avenue in Leamington
  • Sandwich Site on College Avenue and Pickwick site in East Windsor
Health care at each site is provided by a large group of inter-professional health care professionals * a small example of our services includes:
  • Hepatitis C treatment and support
  • Active Aging
  • Diabetes Wellness
  • Leamington & LAMB (Leamington Area Moms & Babies)
 
Active Aging and Diabetes Wellness - Specialized health care, programming and education offered to people affected by a chronic disease such as diabetes, COPD, stroke, heart attack * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups
 
Chronic Disease Self-Management Program - Chronic Disease Self-Management - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions
 
Fall Prevention Screening Clinic - Program is free screening of seniors aged 60 years and older to assess risks related to falls and provide recommendations and referrals within the community to decrease fall risks and reduce injurious falls.

Fees: Most medical services covered by OHIP * some fees for other programs
Application: Call or visit * appointments preferred, walk-ins welcomed * no referral required
Eligibility / Target Population: Vulnerable - Anyone who is in need of care and support with system navigation due to identified risk factors
