Promotes family choices and autonomy and facilitates relationships with families that best meet their needs * an individualized, flexible, family-centred approach will be utilized to facilitate the delivery of support in the community

Eligibility / Target Population:
|
People with intellectual or related disability who are experiencing functional limitations due to disability * must be residents of Ontario and be living with a family
