Service Description: 59-bed long-term care home * residential care for 7 private rooms, 28 semi-private rooms, and 24 basic rooms * 24-hour nursing and personal care * access to a doctor and other health professionals

Risk assessment for falls, assessment for skin and nutrition to enhance quality care. Alzheimer's Gentle Persuasive Approach, and Responsive Behaviour care


FACILITY LIFESTYLE - PERMISSIONS: Smoking - outdoors, no supervision. Alcohol - with doctor's approval

FURNITURE POLICY: May bring own furniture, limited to what room size will allow within safety restrictions. Use facility beds

MEAL PLAN / SERVICE TYPES: On site kitchen prepared family style meals, dining room service, special diets accommodated, tray service when required and family dining available

PHYSICIAN VISIT FREQUENCY: Weekly on site, and as needed

RESTRAINT METHOD: Least restrictive restraint policy, including wheelchair seat belts, wheelchair table tops, bed-rails, environmental - Wanderguard alarm system, and chemical
