Service Description: Number of Beds: 40
Bed Type: Rehab (NRS) - 27 beds (High Intensity), and CC Rehab (CCRS) - 13 beds (Low Intensity)
Program Description:
Patients on this unit have defined rehabilitation goals and are cognitively and physically able to participate in a time-limited, coordinated inter-professional rehabilitation plan of care ranging from low to high intensity through a combined and coordinated use of medical, nursing and allied health skills. Patients may be medically complex with long-term illnesses/disabilities. On admission, patients may have limited physical and/or cognitive capacity due to medical complexity but are believed to have restorative potential.



Admission Criteria:

  • Medically Stable:
    • Clear diagnosis & co-morbidities established,
    • All abnormal lab values have been addressed,
    • All consults and diagnostic tests for purposes of diagnosis or treatment of acute conditions are completed,
    • Medical conditions are stable and can be managed within scope of RN/RPN and do not require daily reassessment of a physician,
    • Vital signs are stable,
    • Feeding and medication needs / routes have been established.
  • Motivated to Participate:
    • Patient is demonstrating willingness and drive to participate in therapy.
  • Demonstrating Progress Towards Goals:
    • Patient is demonstrating improvement towards goals and goal achievement does requires daily access to an interprofessional and coordinated team approach.
  • Tolerating Recommended Therapy Duration:
    • Patient is able to tolerate 15-30 min of therapy 3x / day to 3 hrs / day.

A physician’s referral is required


Fees: Most services covered by OHIP
Application: Referral Process: Program eligibility is assessed and determined by the Unit Charge Nurse in consultation with admitting physician based on admission criteria that align with the Rehabilitative Care Alliance's Bedded Rehabilitative Care Eligibility Criteria. Internal Bluewater Health referrals are initiated within the Health Information System. For external referrals call Patient Flow at 519-464-4400 ext. 5355 between the hours of 8:00AM to 4:00PM.
