Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors and nurse practitioners * access to nurses, nurse educators, dietitians, social workers, pharmacists, health promotor, occupational therapist, chiropractor and massage therapist and respiratory therapist.
