Service Description: This program is provided by the Community Support Centre of Essex County. The program aims to improve quality of life, health and wellness by identifying upstream and downstream opportunities. The program provides holistic, dynamic and unique community circles of care for each person based on the important activities, opportunities, challenges and unmet needs they identify. This program is designed for those feeling lonely and wanting to do more of what they love. Clients set goals, plan action and create the opportunities that you want for themselves.
