Grand Bend Area Community Health Centre

The Grand Bend Area Community Health Centre is a non-profit community governed organization that provides comprehensive primary medical care for our enrolled clients. Our health promotion strategies utilize multidisciplinary teams of health providers and are available to clients as well as everyone in the community.

The centre’s two sites work in collaboration with the Southwest and Erie-St. Clair Local Health Integration Networks along with various agencies and services within the counties of Lambton and Huron. Our ongoing partnership with South Huron Hospital Association enables us to seek out and provide the best care for our clients/patients along with programming for the surrounding communities.

Areas We Serve

Grand Bend Area Community Health Centre has two facilities.

• Grand Bend
• Hensall
The geographic area served by the Grand Bend Area Community Health Centre lies at the junction of the Counties of Huron, Lambton, and Middlesex, and consists of parts of the new municipalities of Bluewater, Lambton Shores, and South Huron. It is bounded by the Ausable Cut on the south, the Village of Crediton on the east, Highway 84 on the north, and Lake Huron to the west.
The Grand Bend Area Community Health Centre provides primary health care services to the individuals who reside or do business in

• Grand Bend (N0M 1T0)
• Hensall (N0M 1X0)

and to individuals in other communities who have previously established relationships with the Centre’s medical staff or have no other access to primary medical services. Such as

• Crediton (N0M 1M0)
• Dashwood (N0M 1N0)
• Kippen (N0M 2E0)
• Port Franks (N0M 2L0)

The GBACHC coordinates its health promotion activities with other community agencies, especially the North Lambton Community Health Centre for services in Port Franks and the southern portion of the area.

Position:  Permanent 0.4 FTE Registered Practical Nurse

Region:  Sarnia – Lambton

Description:  We are seeking a nursing professional who is committed to primary health care and collaborative interprofessional team work, interested in working to their full scope of practice and supporting the System Navigator & OTN Coordinator role. The ideal candidate is passionate about addressing the social determinants of health. The RPN works in partnerships with other members of the Clinical Team to plan coordinate and deliver high quality primary health care services to clients with complex health needs. The successful candidate must have strong common sense; excel at multi-tasking, triage, and problem-solving and have the initiative to proactively support other members of the clinical team in client care.
This position reports to the Director of Health Support Services.

1. Valid Ontario RPN Licence., Experience working in a primary health care setting or a combination of hospital and community settings and or/public health. Experience in venipuncture an asset.
2. Education: Diploma in Practical Nursing
3. Current and valid registration with the College of Nurses of Ontario
4. Member of RPNAO
5. Experience working autonomously within an interprofessional team.
6. Basic computer skills required with an essential focus on electronic medical records knowledge.
7. Thorough knowledge and proficiency in current nursing principles and practices and community based health care.

• Clinical:
- Triage of telephone and walk-in patients.
- Infection control and maintaining medical equipment.
- Maintaining medical inventory.
- Maintaining consult, preparation, treatment and procedure rooms.
- Monitoring and maintaining cold chain control of vaccines.
- Performing and processing of procedures as requested by practitioners such as: venipuncture, drawing up injections, administering injectables, patient preparation and blood pressures, carrying out procedures such as ECG, spirometry, holter monitor, loopers etc.
- Patient instruction and health teaching where applicable.
- Documenting patient interventions.
- Participating in chart reviews if needed.
- At practitioners’ request, informing patients of results and orders.
- Telephoning, faxing and charting renewals of medications to pharmacies upon verbal or written practitioner approval.
- Receiving, processing and distributing lab results, consult notes and letters.

• System Navigation & OTN Coordination Support:
Supports System Navigator & OTN Coordinator in their daily work by identifying and referring patients with high needs by:
- Starting coordinated care plans (CCPs)
- Setting up OTN when needed

• Clerical
- Chart and patient demographic management
- Encountering.
- Third party and uninsured billing.
- Answering telephone inquiries.
- Preparing referral letters.
- Receiving, processing and distributing lab results, consult notes and letters.
- Reception – preparing for, receiving and processing patients and the public.
- Other tasks and duties as assigned by the Director of Health Support Services.
